Awake prone positioning for six hours a day reduces the risk of endotracheal intubation and for eight hours a day reduces the risk of hospital mortality in COVID-19 and acute respiratory failure patients, according to a study published in Crit_Care.

We evaluated the effect of prone positioning on the risk of endotracheal intubation and in-hospital mortality in a cohort of patients admitted to ICU with COVID-19-related acute respiratory failure initially treated with high-flow nasal oxygen and analyzed the effects of different exposure times to AW-PP.We conducted a prospective multicenter cohort study at 6 ICUs of 6 centers in Argentina. Four of these were University Affiliated Centers described in the Additional File.

Once therapy with HFNO was started, participants were encouraged and assisted by the healthcare team to rotate from supine to prone position for as long as possible, taking breaks for personal hygiene and eating. No maximum time limits for prone position were established. Patients who tolerated HFNO for the next 4 h were included in the study. Where prone positioning was not tolerated by patients, they were assisted to remain in the lateral position, alternating right and left decubitus.
