Police Ombudsman investigation found that police had received intelligence about a threat to an unnamed man six months before Jim Donegan was shot dead
The PSNI missed a number of opportunities to identify the subject of a dissident republican threat who was gunned down outside a West Belfast school in 2018, a Police Ombudsman investigation has found.
Members of Donegan’s family told investigators that if he had been made aware of the threat he would have changed his routine so as not to put his son at risk. Anderson noted that police records included previous entries linking Mr Donegan to the personalised registration plate. This information had not been entered into the vehicles section of his police profile.
The investigation found that police made a series of enquiries in a bid to identify the car and the unnamed person referred to in the threat report. When interviewed, the officer who made those enquiries said he had not been aware of Donegan or the previous threats against him. The Ombudsman said disciplinary action would have been recommended against the officer, if it weren't for their retirement before the end of the investigation.
