Several serious incidents were recorded in July and August at the Royal Shrewsbury Hospital and Telford's Princess Royal HospitalA hospital trust has apologised to a patient after the wrong tissue was removed in a surgery.
Two lesions were identified for removal one at a time, but the wrong one was removed in the first operation. The report, presented to the trust's board of directors on Thursday, revealed the patient was "a little anxious" but understood the lesions - abnormal areas of tissue in the body - needed to be removed,The incident was classed as a "never event", the name given to incidents in the NHS considered so serious they should never happen.
Full investigations were underway into the cases which included falls which caused broken bones and head injuries and a "missed opportunity to identify psychosis".
